ARPA-H Awards UpDoc Up to $9.2M to Lead ADVOCATE Autonomous Clinical AI Program

A Federal Pathway to FDA-Authorized Autonomous Clinical AI

While early generative AI in healthcare focused on ambient medical transcription and conversational chatbots, the ADVOCATE initiative represents the federal government’s first direct research-and-development push to build, validate, and deploy an autonomous, FDA-authorized agentic AI clinician-extender.

The program aims to develop 24/7 patient-facing agents that can independently reason across multimodal patient streams—such as connected weight scales, blood pressure cuffs, and EHR records—to actively titrate heart failure medications, adjust diets, recommend physical rehabilitation, and trigger clinician escalation when acute decompensation looms.

UpDoc’s award will fund a 39-month, multi-phase developmental and validation roadmap:

  • Cardiologist-Benchmarked Validation: Independent verification and validation studies directly comparing the diagnostic and therapeutic reasoning of UpDoc’s clinical agents against board-certified cardiologists.
  • FDA Investigational Device Exemption (IDE) Trials: Execution of prospective clinical trials conducted under an FDA IDE framework to generate the safety and efficacy evidence required for formal software-as-a-medical-device (SaMD) authorization.
  • Large-Scale Health System Deployment: Pragmatic scalability trials measuring agent-enabled care against standard clinical care across partner health networks, integrating into enterprise EHR systems.

Evolving Beyond Prescription-Constrained Insulin Titration

The ADVOCATE initiative builds directly on UpDoc’s existing clinical footprint. The company previously secured FDA clearance for an AI-guided insulin management system that automates diabetic dose titration within strict physician-prescribed guardrails.

Through ADVOCATE, UpDoc is advancing its proprietary Clinical Intelligence System from fixed rule sets into multi-step agentic reasoning. Deploying at health systems including Cleveland Clinic, Allegheny Health Network (AHN), and UCSF Health, the underlying architecture pairs autonomous large language model planning with deterministic safety filters, continuous audit logs, and clinician oversight layers to eliminate hallucination risk.

“Nearly one million Americans die from cardiovascular disease each year, many as a result of critical gaps in our current care model,” said Dr. Sharif Vakili, CEO and co-founder of UpDoc and a practicing primary care physician at Stanford Medicine. “Medicine has always been constrained by the number of clinicians and the hours they have in their day dedicated to patient care.
